The Incident Reporting: Recordable / First Aid Application was developed to help you comply with USA OSHA record-keeping requirements. The program allows you to log incidents, keep records, and print all required forms from one place. Entering a new Recordable / First Aid Incident:
Important: An incident can only be entered for an employee if they are already a user / employee. See Add New User / Employee.
Click on Applications
Move cursor down to Incident Reporting
Click on Recordable / First Aid Incidents
Pick a location from the drop down box if multiple locations are available to you
Click [Add New Incident]
Enter information for each section of the incident report
Press the [Save] button at the bottom of the form when finished
Incidents can be edited after they are saved. You can add more details as they become available.
Editing Existing Recordable / First Aid Incidents
Click on Applications
Move cursor down to Incident Reporting
Click on Recordable / First Aid Incidents
To edit an existing incident press the [Edit] button next to the entry
Press the [Save] button at the bottom of the form to save
Printing & Viewing OSHA Forms
Click on Applications
Move cursor down to Incident Reporting
Click on Recordable / First Aid Incidents
Pick a location from the drop down box if multiple locations are available to you
Select desired year from the drop down box
Press [Print Form 300] to view the Log of Work-Related Injuries & Illnesses
Press [Print Form 300A] to view the Summary of Work-Related Injuries & Illnesses
Print the Form 300 or Form 300A using the Adobe Reader tool bar
