Root Cause Administration

Basic Causes (Unsafe Conditions and Behaviors) must be set up in the Root Cause Administration before they will be displayed on the investigation form.

1. Click on Administration
2. Scroll down to Administration ( C )
3. Select Root Cause Administration
4. Click on  [Add New] under Unsafe Conditions
5. Select boxes in the Built in List or type your own Root Causes in the text fields.
6. Click the  [Save] button to save your selections
7. Repeat steps for Behavior(s)
